Client Information Form (Download Now)
   Please complete this form prior to any service. This form will be provided to you when checking in with Invigo for the first time. If you would like to print and have it completed prior to arrival, you may do so.

Parental Consent Form (Download Now)
   This form must be completed for any minor who receives a spa service. This is for clients who are 17 years old or younger. For young minors, a parent must be present in the treatment room. Please inform the spa coordinators of the minor's age when booking appointments.

Club Invigo Signup Form (Download Now)
   This form must be completed when joining Club Invigo. This is NOT a contract, but is a consent form that Invigo Day Spa will be auto-billing the card of your choice each month.   Please do NOT email this form as emailing credit card information is not deemed safe. Please turn it in to the spa in person.
